Staffing Rating
Henry County Health and Rehabilitation Facility earned an overall 4-star rating, which means it performs above average compared to other nursing homes nationally. That overall score is a composite of three separate ratings, and the standout here is staffing, which came in at a perfect 5 stars. That tells you nurses and aides are spending significantly more time with residents than you'd find at a typical facility, which is one of the most meaningful things a family can look for. More staff time generally means faster responses to call lights, more attentive personal care, and a lower chance of residents being left to wait.
The other two pieces are both rated at 3 stars, meaning average. The health inspection score reflects the findings from state inspectors who visit the facility and flag any care or safety issues. An average score here suggests the facility has had some citations, though nothing that puts it in poor-standing territory. The quality measures score is based on clinical outcomes like things such as falls, pressure ulcers, and whether residents' conditions are being well managed over time, and average here means results are in line with most other facilities. The strong staffing rating is what pushes the overall score above average, so families should feel reasonably confident about day-to-day care while also asking the facility directly about any recent inspection findings.

Questions to ask when visiting
1. How many residents does each certified nursing assistant care for during the day shift, and does that number change at night or on weekends?
2. How long have your nursing staff and CNAs been working here on average, and what does your turnover look like over the past year?
3. If my family member falls or has a medical emergency at 2 a.m., walk me through exactly what happens from the moment staff are alerted.
4. How do you handle a resident who repeatedly refuses medication, meals, or personal care, and who makes the call on next steps?
5. What does a typical Tuesday look like for a resident who is mobile and social versus one who is mostly in their room?
6. Can you show me the most recent state inspection report, and can you explain any deficiencies that were cited and what changed because of them?

About CMS Star Ratings
CMS assigns 1–5 star ratings to Medicare and Medicaid-certified nursing homes based on three domains: health inspections, staffing levels, and quality measures. The overall rating is a weighted composite.
A 5-star overall rating does not guarantee excellence in every domain — inspect each sub-rating independently. Data is updated quarterly.
