Staffing Rating
Arlington Rehabilitation & Healthcare Center earns an overall 4-star rating, which places it above average compared to nursing homes nationally. That overall score is a composite of three separate ratings, and it's worth knowing that the pieces don't all tell the same story here. The health inspection rating is also 4 stars, which is the strongest piece of the puzzle. It reflects that state inspectors found relatively few problems during their visits, and when issues were cited, they tended to be less severe. For families, that's a meaningful sign that the facility is generally meeting care standards and holding up well under outside scrutiny.
The staffing and quality measures ratings both come in at 2 stars, meaning below average, and those are worth paying attention to. The staffing score reflects that nurses and aides are spending fewer hours per resident than what's typical at comparable facilities. In practical terms, that can mean residents wait longer for assistance or have less one-on-one time with staff. The quality measures rating is based on 15 clinical indicators, things like whether residents are experiencing falls, pressure ulcers, declines in mobility, or other health changes. A below-average score there suggests that resident health outcomes may not be keeping pace with what you'd see at better-performing facilities. If your loved one has complex medical needs or requires close daily attention, these two lower scores are important to ask the facility about directly before making a decision.

Questions to ask when visiting
1. How many residents does each certified nursing assistant care for during the day shift, and does that number change at night or on weekends?
2. How long have your nursing staff and aides been working here on average, and what is your turnover rate over the past year?
3. If my family member has a fall or a medical change overnight, what is the exact process for notifying me, and how quickly can I expect that call?
4. Can you walk me through what a typical Tuesday looks like for a resident who has limited mobility and does not have many outside visitors?
5. In the past 12 months, have you had any state inspection citations related to neglect, medication errors, or understaffing, and can I see the most recent survey report?
6. If my family member is unhappy here or I have a complaint about their care, who do I speak with first, and what happens if that does not resolve the problem?

About CMS Star Ratings
CMS assigns 1–5 star ratings to Medicare and Medicaid-certified nursing homes based on three domains: health inspections, staffing levels, and quality measures. The overall rating is a weighted composite.
A 5-star overall rating does not guarantee excellence in every domain — inspect each sub-rating independently. Data is updated quarterly.
