Staffing Rating
Westside Terrace Health & Rehabilitation Center holds a 4-star overall rating from CMS, which means it performs above average compared to nursing homes across the country. That overall rating is built from three separate scores, and in this case all three are pulling in the same direction, which is a good sign.
Each piece of the rating tells you something different about the facility. The 4-star Health Inspection rating means that when state inspectors visited and reviewed the facility, they found fewer or less serious problems than most nursing homes. The 4-star Staffing rating tells you that nurses and aides are spending more time with residents than the national average, which in practical terms means your loved one is less likely to wait long for help and more likely to get consistent, attentive care. The 4-star Quality Measures rating reflects how residents are actually doing physically and clinically, covering things like fall rates, pressure wounds, pain management, and mobility. Scoring above average here suggests the facility is delivering solid day-to-day care outcomes. Taken together, these ratings paint a reasonably consistent picture of a facility that is performing well across the board, not just in one area while struggling in another.

Questions to ask when visiting
1. How many residents is each certified nursing assistant responsible for during the day shift, and does that number change at night or on weekends?
2. If my loved one has a concern or feels unsafe, who specifically do they talk to, and how quickly can we expect a response?
3. Can you show me the most recent state inspection report, and walk me through any deficiencies that were cited and what you did to fix them?
4. How do you handle situations where a resident's condition changes, such as a sudden fall or signs of a new infection, and how soon would you contact the family?
5. What does a typical day look like for a resident who needs help with meals, bathing, and getting around, and how much of that care is handled by the same staff members each week?
6. What is your current staff turnover rate, and how long have most of your nursing assistants and nurses been working at this facility?

About CMS Star Ratings
CMS assigns 1–5 star ratings to Medicare and Medicaid-certified nursing homes based on three domains: health inspections, staffing levels, and quality measures. The overall rating is a weighted composite.
A 5-star overall rating does not guarantee excellence in every domain — inspect each sub-rating independently. Data is updated quarterly.
