Staffing Rating
Arabella Health and Wellness of Fairhope holds an overall 4-star rating from CMS, which means it performs above average compared to nursing homes nationally. That overall score is a composite of three separate ratings, and the standout here is the Quality Measures rating, which comes in at a perfect 5 stars. This score is based on 15 clinical and physical health outcomes for residents, things like fall rates, pressure wounds, and how well residents maintain their ability to move and function. A 5-star score there is a strong signal that the day-to-day health of residents is being well managed. The other two pieces of the picture are both average, at 3 stars each. The Health Inspection rating reflects what state inspectors found during their visits, including the number and seriousness of any citations the facility received. Three stars here means Arabella is in the middle of the pack, so it is worth asking the facility directly about any recent inspection findings. The Staffing rating, also 3 stars, is based on how many hours of nursing care residents receive relative to the number of people living there. Average staffing is not a red flag, but it does mean families should check whether staffing levels feel adequate during a visit. Together, these scores suggest a facility where resident health outcomes are genuinely impressive, even if inspection history and staffing are more typical for the industry.

Questions to ask when visiting
1. How many residents does each certified nursing assistant care for during a typical day shift and a typical night shift? 2. How often do residents see the same aides and nurses throughout the week, and what is the facility's current staff turnover rate? 3. What happens when a resident has a fall, and can you walk me through exactly what steps your staff takes in the first hour after an incident like that? 4. If my loved one has a complaint or feels uncomfortable about something, who do they talk to, and how do I find out it was addressed? 5. What does a regular weekday look like for a resident who doesn't have many scheduled activities or therapy appointments? 6. Can I see the most recent state inspection report, and is there anything in it you want to explain to me before I read it? About CMS Star Ratings

CMS assigns 1–5 star ratings to Medicare and Medicaid-certified nursing homes based on three domains: health inspections, staffing levels, and quality measures. The overall rating is a weighted composite.

A 5-star overall rating does not guarantee excellence in every domain — inspect each sub-rating independently. Data is updated quarterly.
