Staffing Rating
Haleyville Health Care Center earned an overall 4-star rating from CMS, which puts it above average compared to nursing homes nationwide. That overall score is built from three separate ratings, and it's worth knowing how each one shakes out before you draw any conclusions.
The strongest piece here is the Health Inspection rating, which comes in at a perfect 5 stars. That means state inspectors found very few, if any, serious problems during their visits, which is a real positive sign for how the facility is being run day to day. Staffing lands at 4 stars, meaning residents are generally getting more hands-on nursing time than you'd find at a typical facility, which tends to translate to better responsiveness and more attentive care. The area that stands out as a concern is Quality Measures, which sits at just 1 star. This rating looks at 15 clinical outcomes, things like whether residents are developing pressure wounds, losing too much weight, or experiencing falls, and a 1-star score here means those outcomes are running below what you'd expect. That gap is worth a direct conversation with the facility. Ask them how they're working to improve those outcomes and what specific measures they're tracking internally. The inspection and staffing scores show real strengths, but the quality measures rating is a signal that families shouldn't overlook.

Questions to ask when visiting
1. How many residents does each certified nursing assistant care for during the day shift, and how does that number change at night and on weekends?
2. How long have your charge nurses and CNAs been working here on average, and what does your staff turnover look like over the past year?
3. If my loved one falls or has a medical emergency overnight, walk me through exactly what happens from the moment it occurs to when the family gets a call.
4. Can I see the most recent state inspection report, and can you tell me how you addressed any deficiencies listed in it?
5. What does a typical weekday look like for a resident who does not leave their room very often, and how do staff make sure that person stays engaged and comfortable?
6. If my loved one is unhappy here or has a complaint, who specifically do they or I talk to, and can you give me an example of a complaint that was raised and how it was resolved?

About CMS Star Ratings
CMS assigns 1–5 star ratings to Medicare and Medicaid-certified nursing homes based on three domains: health inspections, staffing levels, and quality measures. The overall rating is a weighted composite.
A 5-star overall rating does not guarantee excellence in every domain — inspect each sub-rating independently. Data is updated quarterly.
