Staffing Rating
Orchard Rehabilitation & Healthcare Center earns an overall 4 out of 5 stars, which puts it above average compared to other nursing facilities nationwide. That overall number is a composite of three separate ratings, so it helps to look at each piece individually to get the full picture.
The strongest area here is health inspections, where Orchard scores a perfect 5 stars. That means state inspectors found very few problems, or none of serious concern, during their visits, which is a good sign that the facility is being run safely and up to code. Staffing also comes in strong at 4 stars, meaning nurses and aides are spending more time with residents than you'd typically see at comparable facilities. More staff time generally translates to faster responses, more attentive care, and less stress on the residents. The area that deserves a closer look is quality measures, which sits at just 1 star. This rating tracks actual clinical outcomes, things like whether residents are experiencing falls, pressure sores, or declines in mobility and daily functioning. A 1-star score here means those outcomes are running well below average, and that is worth asking the facility about directly. It does not erase the positives, but families should go in with specific questions about how the facility monitors and addresses resident health over time.

Questions to ask when visiting
1. How many residents does each certified nursing assistant care for during a typical day shift, and does that number change at night or on weekends?
2. How long have most of your direct care staff been working here, and what do you do to keep turnover low?
3. If my loved one has a fall or a medical concern overnight, who is responsible for responding and how quickly does a nurse get involved?
4. Can you walk me through what a typical Tuesday looks like for a resident who doesn't have many visitors and prefers to stay in their room?
5. How do you handle it when a resident or family member complains about care, and can you give me an example of a complaint that led to a real change here?
6. What is your current star rating with Medicare, and if it dropped at any point in the last two years, what caused it and what changed as a result?

About CMS Star Ratings
CMS assigns 1–5 star ratings to Medicare and Medicaid-certified nursing homes based on three domains: health inspections, staffing levels, and quality measures. The overall rating is a weighted composite.
A 5-star overall rating does not guarantee excellence in every domain — inspect each sub-rating independently. Data is updated quarterly.
