Staffing Rating
Lineville Health and Rehabilitation earned a 5-star overall rating from CMS, which is the highest possible score and puts it in a strong position compared to most nursing homes across the country. A big part of that comes from its perfect 5-star Health Inspection rating, which means state inspectors found very few, if any, serious problems during their visits. That is genuinely reassuring for families, because health inspections look at real conditions inside the facility and can flag issues with care practices, safety, and resident rights.
The other two pieces of the picture are more middle-of-the-road. Staffing comes in at 3 stars, meaning the number of nursing hours per resident is about average for the industry. That is not a red flag, but it is worth asking the facility about how they schedule staff and how they handle busy periods. Quality Measures also sits at 3 stars, reflecting average performance across 15 clinical outcomes like fall rates, pressure ulcers, and how well residents maintain their physical abilities. Together, the strong inspection score carries a lot of weight in pulling the overall rating up to 5 stars, so families should feel good about the oversight and compliance side of things while still asking thoughtful questions about day-to-day staffing levels and clinical outcomes during a tour.

Questions to ask when visiting
1. How many residents does each certified nursing assistant typically care for during a day shift, and does that number change at night or on weekends?
2. How long have most of your nurses and aides been working here, and what does staff turnover look like over the past year?
3. If my loved one falls or has a medical emergency overnight, what is the step-by-step process for notifying our family, and how quickly does that call or message happen?
4. Can I see the most recent state inspection report, and can you walk me through any deficiencies that were cited and what changed as a result?
5. What does a typical weekday look like for a resident, from morning care through the evening, and how much of that time involves activities, meals with others, or time outside?
6. If my loved one has a concern or complaint, who do they talk to, and what happens if they are not comfortable speaking up on their own?

About CMS Star Ratings
CMS assigns 1–5 star ratings to Medicare and Medicaid-certified nursing homes based on three domains: health inspections, staffing levels, and quality measures. The overall rating is a weighted composite.
A 5-star overall rating does not guarantee excellence in every domain — inspect each sub-rating independently. Data is updated quarterly.
