Staffing Rating
Southland Nursing Home holds an overall 5-star rating from CMS, which is the highest possible score and puts it well above most nursing homes nationwide. That overall rating is a composite of three separate scores, and understanding each one gives you a clearer picture of what daily life there might look like for your loved one. The health inspection rating comes in at 4 stars, meaning state inspectors found fewer or less serious problems than at most facilities. That's a strong sign that the home is generally meeting safety and care standards. The quality measures rating is a perfect 5 stars, which reflects how residents are actually doing physically and clinically across 15 tracked health outcomes, things like fall rates, pressure wounds, and pain management. In practical terms, that means residents at Southland tend to fare better on meaningful health markers compared to the vast majority of other nursing homes. The one area worth keeping in mind is staffing, which sits at 3 stars, or average. This rating looks at how many hours of nursing care each resident receives relative to the number of residents in the building. Average isn't a red flag, but it does mean staffing levels aren't a standout strength the way clinical outcomes are. It's worth asking the facility directly about nurse-to-resident ratios and how they handle staffing on nights and weekends. Overall though, Southland's combination of strong inspection results and excellent health outcomes, backed by an overall 5-star rating, suggests this is a facility that tends to deliver solid care in practice.

Questions to ask when visiting
1. How many residents does each certified nursing assistant care for during a typical day shift and night shift, and has that ratio changed in the past year? 2. What percentage of your nursing staff are permanent employees versus agency or temporary workers, and how often do residents see the same faces from week to week? 3. Can you walk me through exactly what happens when a resident falls, has a medical emergency, or shows signs of a sudden health change, from the moment staff notice something is wrong to when the family is notified? 4. How do you handle a resident who refuses a meal, skips activities for several days in a row, or seems withdrawn, and who is responsible for following up on that? 5. What does a typical Tuesday look like for a resident who needs help with bathing, dressing, and getting to meals, and how much of that routine does the resident get to control? 6. Can you show me your most recent state inspection report, and can you explain the deficiencies that were cited and what specific changes were made afterward? About CMS Star Ratings

CMS assigns 1–5 star ratings to Medicare and Medicaid-certified nursing homes based on three domains: health inspections, staffing levels, and quality measures. The overall rating is a weighted composite.

A 5-star overall rating does not guarantee excellence in every domain — inspect each sub-rating independently. Data is updated quarterly.
