Staffing Rating
Palm Gardens Health and Rehabilitation earned an overall 5-star rating from CMS, which is the highest possible score and puts it well above most nursing homes nationally. That top-line number is a composite built from three separate ratings, so it helps to understand what each one is actually measuring. The 5-star Health Inspection rating is a strong signal for families, because it means state inspectors found very few problems during their visits, and any issues they did find were minor. That kind of record suggests the facility takes compliance and resident safety seriously on a day-to-day basis. The 4-star Staffing rating means nurses and aides are spending more time with residents than the average facility, though not quite at the very top tier. More staff time generally translates to quicker responses to call lights, more attentive care, and less of the rushed feeling that can be a concern in understaffed homes.
The one area worth a closer look is the 3-star Quality Measures rating, which lands right at average. This score is based on 15 clinical and physical health outcomes for residents, things like fall rates, pressure wounds, and how residents are managing pain or mobility. Average is not bad, but it does mean other highly rated facilities in the area may be seeing somewhat better health outcomes for their residents. For families, this is worth asking about directly during a tour. Still, the overall picture here is genuinely positive. The combination of a spotless inspection record and strong staffing is exactly what you want to see, and the quality measures score keeps things grounded in a realistic way.

Questions to ask when visiting
1. How many residents does each certified nursing assistant care for during the day shift, and does that number change at night or on weekends?
2. How long have your charge nurses and CNAs been working here on average, and what does your staff turnover look like over the past year?
3. If my loved one has a complaint or feels unsafe, who do they go to, and can you walk me through exactly what happens after that?
4. How often does the same staff member work with my loved one week to week, or do you rotate people across different residents regularly?
5. What does a typical day look like for a resident who needs help with meals, bathing, and getting around, from the time they wake up until they go to bed?
6. Can you show me your most recent state inspection report, and how did you address any deficiencies that were cited?

About CMS Star Ratings
CMS assigns 1–5 star ratings to Medicare and Medicaid-certified nursing homes based on three domains: health inspections, staffing levels, and quality measures. The overall rating is a weighted composite.
A 5-star overall rating does not guarantee excellence in every domain — inspect each sub-rating independently. Data is updated quarterly.
