Staffing Rating
Sylacauga Health and Rehab Services earns an overall 5-star rating from CMS, which is the highest possible score and puts this facility well above average nationally. That overall rating is a composite picture built from three separate categories, each measuring something different about how a facility operates and cares for its residents. On health inspections, the facility scores 4 out of 5 stars, meaning state surveyors found fewer or less serious citations than most nursing homes when they came in to inspect. That is a reassuring sign that the facility is generally meeting care and safety standards. Staffing also comes in at 4 stars, which tells you that nurses are spending more time with residents than the average facility, and that matters day to day when your loved one needs help or something changes with their health. The standout here is the quality measures rating, a perfect 5 stars. That score reflects how residents are actually doing across 15 clinical markers, things like rates of infections, falls, pressure wounds, and whether residents are maintaining their physical function. A 5-star score there means the outcomes for residents are much better than average, and that is often the most meaningful number for families to pay attention to.

Questions to ask when visiting
1. How many residents does each certified nursing assistant care for during the day shift, and does that number change at night or on weekends? 2. How long have most of your floor nurses and aides been working here, and what does your staff turnover look like over the past year? 3. If my loved one falls or has a medical emergency overnight, walk me through exactly what happens from the moment staff find out. 4. Can I come back for an unannounced visit at a different time of day, and is there any time I would not be allowed in? 5. How do you handle a resident who refuses medications or meals, and who do you contact in the family when something like that happens? 6. What does a typical Tuesday look like for a resident who is not very mobile - what activities are available, who checks on them, and how much time do they spend in their room alone? About CMS Star Ratings

CMS assigns 1–5 star ratings to Medicare and Medicaid-certified nursing homes based on three domains: health inspections, staffing levels, and quality measures. The overall rating is a weighted composite.

A 5-star overall rating does not guarantee excellence in every domain — inspect each sub-rating independently. Data is updated quarterly.
