Staffing Rating
Cottage of the Shoals earned an overall 5-star rating from CMS, which is the highest possible score. That overall rating is a composite built from three separate scores, each measuring something different about how a facility operates. The health inspection rating came in at 4 stars, meaning state inspectors found fewer or less serious citations than most facilities, which is a good sign that the home is being run safely and according to regulations. The quality measures rating is a perfect 5 stars, reflecting that residents here tend to fare very well across 15 clinical health indicators, things like how often residents experience falls, pressure wounds, or declines in daily functioning. That's meaningful because it speaks directly to the care people are actually receiving day to day. The one area worth paying attention to is staffing, which landed at 2 stars, meaning the facility has fewer nursing hours per resident than most. This doesn't necessarily reflect poorly on the staff themselves, but it does suggest that nurses and aides may be stretched thinner than at comparable homes.
For families, the big picture here is mostly encouraging. The strong health inspection and exceptional quality outcomes suggest that residents are being well cared for, even with the staffing numbers running on the leaner side. That said, it's worth asking the facility directly about their staffing levels during your visit, particularly for a loved one who needs more hands-on attention throughout the day. Overall ratings can sometimes mask one weaker area, and in this case the staffing score is the piece of the puzzle that deserves a closer look.

Questions to ask when visiting
1. How many residents does each certified nursing assistant typically care for during the day shift, and how does that change on nights and weekends?
2. How long have your charge nurses and CNAs been working here, and what does your turnover look like over the past year?
3. What happens if my loved one falls or has a medical emergency in the middle of the night - who responds, and how quickly?
4. Can you walk me through what a typical Tuesday looks like for a resident who doesn't have many scheduled therapies or appointments?
5. How do you handle complaints from residents or family members, and can you give me an example of a complaint that led to a real change here?
6. Has this facility been cited for any deficiencies in the past two state inspections, and if so, what did you do to fix them?

About CMS Star Ratings
CMS assigns 1–5 star ratings to Medicare and Medicaid-certified nursing homes based on three domains: health inspections, staffing levels, and quality measures. The overall rating is a weighted composite.
A 5-star overall rating does not guarantee excellence in every domain — inspect each sub-rating independently. Data is updated quarterly.
