Staffing Rating
The Legacy Rehab & Care Center holds an overall 5-star rating from CMS, which is about as strong as it gets. That top score is a composite of three separate ratings, and it's worth understanding what each one tells you. The health inspection rating is a perfect 5 stars, meaning state inspectors found very few, if any, serious problems during their visits. That's a meaningful signal about day-to-day operations and accountability. The quality measures rating comes in at 4 stars, which is above average across 15 clinical indicators like fall rates, pressure wounds, and how well residents maintain their physical function. In practical terms, that means residents here tend to fare better health-wise than at most facilities. The staffing rating is 3 stars, or average, reflecting that nurse hours per resident fall in the middle of the range compared to other nursing homes. That doesn't necessarily mean care is lacking, but it's worth asking the facility how they staff on nights and weekends specifically.
It's a little unusual to see a 5-star overall rating alongside a 3-star staffing score, so it's fair to wonder how those pieces fit together. CMS weighs health inspections heavily in the overall calculation, so the facility's clean inspection record gives it a strong foundation. The solid quality outcomes score helps as well. What this means for your family is that the facility has a strong track record with regulators and produces good clinical results for residents, even if the nurse-to-resident ratio is not exceptional. Asking the staff directly about how they handle busy periods or unexpected needs is a reasonable follow-up question during your visit.

Questions to ask when visiting
1. How many residents does each certified nursing assistant care for during a typical day shift, and does that number change at night or on weekends?
2. How long have most of your nursing staff been working here, and what do you do to keep staff from leaving?
3. If my loved one has a complaint or feels unsafe, who do they talk to, and can you walk me through exactly what happens after that conversation?
4. How often does the same aide or nurse work with the same resident, so my family member is seeing familiar faces rather than a different person every day?
5. What happened the last time there was a fall or a medication error here, and what did the facility change as a result?
6. Can I come back for an unannounced visit at a different time of day, including on a weekend, and walk through the common areas and dining room?

About CMS Star Ratings
CMS assigns 1–5 star ratings to Medicare and Medicaid-certified nursing homes based on three domains: health inspections, staffing levels, and quality measures. The overall rating is a weighted composite.
A 5-star overall rating does not guarantee excellence in every domain — inspect each sub-rating independently. Data is updated quarterly.
