Staffing Rating
Oasis Pavilion Nursing & Rehabilitation Center holds an overall 4-star rating from CMS, meaning it performs above average compared to nursing homes across the country. That overall score is built from three separate ratings that each measure something different, so it helps to look at what's driving it. The Health Inspection rating is 4 stars, which reflects above-average results from state inspections. Inspectors look at how many citations a facility received and how serious those issues were, so a strong score here suggests the facility has been meeting care and safety standards without major violations. The Quality Measures rating is also 4 stars, meaning residents at Oasis Pavilion tend to see above-average outcomes across 15 clinical indicators like fall rates, wound care, and mobility. The one area worth keeping an eye on is Staffing, which comes in at 3 stars, or average. This rating is based on how many hours of nursing care residents receive relative to the number of people living there. Average isn't a red flag, but it does mean families may want to ask how staffing is handled during nights and weekends, or during periods when the facility is at higher capacity.

Questions to ask when visiting
1. How many residents does each certified nursing assistant care for during the day shift, and how does that change at night and on weekends? 2. How long have your most recent director of nursing and administrator been in their roles here, and what is your average staff turnover rate over the past year? 3. Can you walk me through exactly what happens if a resident falls, has a medical emergency, or shows signs of a sudden health change, from the moment it's noticed to when the family is contacted? 4. What does a typical weekday look like for a resident who needs memory care or significant physical assistance, from morning wake-up through the evening? 5. Has this facility been cited for any deficiencies in the past two state inspections, and if so, what specific steps did you take to correct them? 6. If my loved one has a complaint or feels they are not being treated with dignity, who do they or I contact, and can you show me how that process has worked in a real situation? About CMS Star Ratings

CMS assigns 1–5 star ratings to Medicare and Medicaid-certified nursing homes based on three domains: health inspections, staffing levels, and quality measures. The overall rating is a weighted composite.

A 5-star overall rating does not guarantee excellence in every domain — inspect each sub-rating independently. Data is updated quarterly.
