Staffing Rating
Citadel Post Acute earned an overall 5-star rating from CMS, which is the highest possible score and puts it well above most facilities nationally. That overall rating is built from three separate scores, each measuring something different. The health inspection rating comes in at 4 stars, meaning state inspectors found fewer and less serious problems here than at most facilities, which is a good sign that the day-to-day environment is well managed. The quality measures rating is a perfect 5 stars, reflecting strong outcomes across 15 clinical benchmarks like wound care, fall rates, and medication management. That tells you residents are generally faring well physically, which is often what families care most about. The one area to keep in mind is staffing, which sits at 3 stars, meaning the ratio of nurse hours to residents is about average. It is not a red flag, but it is worth asking the facility directly about how they schedule staff and cover busy times.
The good news is that the two areas with the most direct impact on resident health, clinical outcomes and inspection results, are both strong. The average staffing score keeps the overall picture from being perfect on every front, but it did not prevent Citadel from reaching that top overall rating. For families, this profile suggests a facility that delivers solid clinical care and passes inspections well, while staffing levels are typical of what you would find at many reputable nursing homes. It is always smart to visit in person and ask staff how they handle nights and weekends, just to get a feel for the real-world experience.

Questions to ask when visiting
1. How many residents does each certified nursing assistant care for during the day shift, and does that number change at night or on weekends?
2. How long have your charge nurses and CNAs been working here on average, and what does your staff turnover look like over the past year?
3. If my loved one falls or has a medical emergency in the middle of the night, walk me through exactly what happens from the moment it occurs to when the family gets a call.
4. Can I come back for an unannounced visit at a different time of day, and is there anything that would prevent me from doing that?
5. How do you handle a resident who refuses a meal, starts showing signs of depression, or seems to be in pain but can't communicate it clearly?
6. What does a typical Tuesday look like for a resident who doesn't have family visiting that day, from the time they wake up to the time they go to bed?

About CMS Star Ratings
CMS assigns 1–5 star ratings to Medicare and Medicaid-certified nursing homes based on three domains: health inspections, staffing levels, and quality measures. The overall rating is a weighted composite.
A 5-star overall rating does not guarantee excellence in every domain — inspect each sub-rating independently. Data is updated quarterly.
