Staffing Rating
Mission Palms Post Acute holds a 4-star overall rating from CMS, which puts it above average compared to other nursing facilities nationwide. That overall number is built from three separate scores, and the picture here is interesting because one area really stands out. The Quality Measures rating is a perfect 5 stars, meaning the facility performs much better than average across 15 clinical and physical health outcomes tracked for residents, things like how well the facility manages pain, prevents pressure sores, or maintains residents' mobility. That is a meaningful signal for families, because it reflects what is actually happening to people living there day to day.
The Health Inspection and Staffing ratings each land at 3 stars, which is average. On the inspection side, that means state surveyors found a typical number of citations during their visits, nothing unusually alarming, but not a spotless record either. On staffing, the facility provides a fairly standard number of nurse hours per resident, so you would not expect to see it shorthanded, but it is also not going above and beyond in that area. The strong Quality Measures score pulls the overall rating up to 4 stars, and that combination tells a reasonable story: a facility that may not be flashy on paper in every category, but where the real-world health outcomes for residents are genuinely impressive.

Questions to ask when visiting
1. How many residents does each certified nursing assistant care for during the day shift, and how does that change at night and on weekends?
2. How long have your charge nurses and CNAs been working here on average, and what is your staff turnover rate over the past year?
3. Can you walk me through exactly what happens if a resident falls or has a medical emergency in the middle of the night?
4. How do you handle a resident who resists care, gets confused, or becomes upset, and who makes those calls?
5. What does a typical weekday actually look like for a resident who needs help with meals, bathing, and getting around?
6. Can I see the most recent state inspection report, and can you explain any deficiencies that were cited?

About CMS Star Ratings
CMS assigns 1–5 star ratings to Medicare and Medicaid-certified nursing homes based on three domains: health inspections, staffing levels, and quality measures. The overall rating is a weighted composite.
A 5-star overall rating does not guarantee excellence in every domain — inspect each sub-rating independently. Data is updated quarterly.
