Staffing Rating
Peoria Post Acute and Rehabilitation holds a 4-star overall rating from CMS, which means it performs above average when all the pieces are weighed together. That overall number is a composite of three separate scores, so it helps to understand what each one actually tells you. The health inspection rating comes in at 4 stars, meaning state inspectors found fewer or less serious problems here than at most facilities - a good sign that the basics of safety and compliance are being met. The quality measures rating is a perfect 5 stars, which reflects how residents are actually doing physically and clinically across 15 different health outcomes, things like whether residents are experiencing falls, pressure wounds, or declines in mobility. That score is arguably the most meaningful one for families, because it speaks directly to the care results residents experience day to day.
The one area worth paying close attention to is staffing, which sits at just 1 star - much below average. This score is based on how many hours of nursing care each resident receives, and a low number here can mean nurses and aides are stretched thin. It does not automatically mean residents receive poor care, and the strong quality measures score suggests the facility is achieving good outcomes despite this, but it is a real conversation to have with the staff before making a decision. Ask about typical staffing levels on the shift your loved one would be on, and how the facility handles nights and weekends. Going in with that question ready will tell you a lot.

Questions to ask when visiting
1. How many residents does each certified nursing assistant care for during the day shift, and does that ratio change at night or on weekends?
2. How long have your current nurses and aides been working here, and what does your staff turnover look like over the past year?
3. If my loved one falls or has a medical emergency overnight, walk me through exactly what happens and who responds first.
4. How do you handle a resident who refuses a bath, skips meals, or pushes back on their care routine without just overriding what they want?
5. Can I see the most recent state inspection report, and can you tell me what you changed after the last citation or complaint?
6. What does a typical Tuesday look like for a resident who is mobile but has memory loss, from the time they wake up until they go to bed?

About CMS Star Ratings
CMS assigns 1–5 star ratings to Medicare and Medicaid-certified nursing homes based on three domains: health inspections, staffing levels, and quality measures. The overall rating is a weighted composite.
A 5-star overall rating does not guarantee excellence in every domain — inspect each sub-rating independently. Data is updated quarterly.
