Staffing Rating
North Mountain Medical and Rehabilitation Center holds an overall 5-star rating from CMS, which is the highest possible score. That overall rating pulls together three separate pieces: health inspections, staffing levels, and quality measures. Two of those three areas are genuinely exceptional here. The health inspection rating of 5 stars means state inspectors found very few, if any, serious problems during their visits, which is a strong sign that the facility is being run carefully and safely. The quality measures rating of 5 stars reflects excellent outcomes across 15 clinical markers, things like how well the facility manages pain, prevents infections, or helps residents maintain their physical functioning. Those outcomes speak directly to the care residents actually receive day to day. The one area worth a closer look is staffing, which comes in at 2 stars, meaning nurse hours per resident are below average compared to other facilities. This does not cancel out the strong performance elsewhere, and the high quality outcomes suggest the staff that is present is doing good work. But for families whose loved ones have complex medical needs or require a lot of hands-on attention, it is a fair question to raise directly with the facility. Ask how they handle staffing during nights and weekends, and how they manage during busy periods. The overall 5-star rating is genuinely impressive, and this is still a high-performing facility, but going in with clear eyes about staffing will help you make the best decision for your family.

Questions to ask when visiting
1. How many residents does each certified nursing assistant care for during the day shift, and does that number change at night or on weekends? 2. How long have most of your nursing staff been working here, and what does your turnover rate look like compared to last year? 3. If my loved one has a fall or a medical change overnight, what exactly happens and who calls me, how soon? 4. Can I come back tomorrow without calling ahead first, and are there any times of day when family visits are discouraged? 5. What does a typical day actually look like for a resident who needs help with meals, bathing, and getting around, from the time they wake up to when they go to sleep? 6. Has this facility been cited for any deficiencies in the last two state inspections, and can I see those reports before I leave today? About CMS Star Ratings

CMS assigns 1–5 star ratings to Medicare and Medicaid-certified nursing homes based on three domains: health inspections, staffing levels, and quality measures. The overall rating is a weighted composite.

A 5-star overall rating does not guarantee excellence in every domain — inspect each sub-rating independently. Data is updated quarterly.
