Staffing Rating
Plaza Healthcare earned an overall 5-star rating from CMS, which is the highest possible score and puts it in the top tier of nursing facilities nationwide. That overall score is a composite picture built from three separate ratings, each measuring something different about how the facility operates. On health inspections, Plaza earned 4 stars, meaning state inspectors found fewer and less serious problems there than at most facilities. On quality measures, the facility hit the top mark of 5 stars, which reflects strong outcomes across 15 clinical indicators tracking things like how residents manage pain, avoid infections, maintain mobility, and stay out of the hospital. That combination tells you the care residents actually receive day-to-day is producing genuinely good results.
The one area worth noting is staffing, which sits at 3 stars, or average. This rating looks at how many nursing hours per day each resident receives relative to the number of residents in the building. Average isn't alarming, but it does mean families may want to ask the facility directly about staffing levels, particularly on evenings and weekends when coverage can thin out at many facilities. The good news is that despite the middle-of-the-road staffing score, the quality measures and inspection results both trend well, which suggests the staff they do have is working effectively. Overall, Plaza Healthcare presents a strong picture, and that 5-star composite rating reflects a facility performing well above average by most measures families should care about.

3. If my family member falls or has a medical emergency overnight, who responds and how quickly can they reach a nurse?
4. Can you walk me through what a typical Tuesday looks like for a resident who doesn't have any scheduled therapy that day?
5. How do you handle it when a resident or family member makes a complaint, and can you give me an example of a change you made because of one?
6. What is your current state inspection history, and have you had any citations in the past two years related to resident safety or staffing?

About CMS Star Ratings
CMS assigns 1–5 star ratings to Medicare and Medicaid-certified nursing homes based on three domains: health inspections, staffing levels, and quality measures. The overall rating is a weighted composite.
A 5-star overall rating does not guarantee excellence in every domain — inspect each sub-rating independently. Data is updated quarterly.
