Staffing Rating
The Center at Tucson earned an overall 5-star rating from CMS, which is the highest possible score and reflects how the facility performs across three separate areas that are then combined into one composite picture. On health inspections, they also received a perfect 5 stars, meaning state inspectors found very few or no serious citations during their reviews. That's a strong sign that the facility is being run safely and that problems are caught and addressed before they become bigger concerns for residents. Their Quality Measures rating is also a 5, which looks at 15 clinical indicators like how well the staff manages pain, prevents pressure sores, and maintains residents' physical functioning. Scores like that suggest residents are receiving attentive, hands-on care day to day. The one area that sits slightly lower is Staffing, which earned 4 stars, meaning it's above average but not quite at the top. This rating reflects how many hours of nursing care each resident receives on a typical day. A 4 here is still a good sign, and it doesn't raise any red flags, but families who have a loved one with more complex medical needs may want to ask the facility directly about nurse-to-resident ratios and how staffing levels are maintained on nights and weekends. Overall, this is a very strong profile, and for most families, a 5-star composite backed by near-perfect scores across the board is a genuinely reassuring place to start.

Questions to ask when visiting
1. How many residents does each certified nursing assistant typically care for during a day shift, and does that number change significantly at night or on weekends? 2. How long have your charge nurses and CNAs been working at this facility on average, and what does your staff turnover look like over the past year? 3. If my loved one has a fall or a sudden change in condition overnight, walk me through exactly what happens and who makes the call to notify family? 4. Can you show me the most recent state inspection report, and can you explain what was cited and what you did to fix it? 5. How does a typical weekday look for residents who are mobile and social, and what happens on weekends when activity staffing is usually lighter? 6. If my loved one is unhappy here or I have a serious concern about their care, who do I go to first and what has the facility actually done in the past when families raised complaints? About CMS Star Ratings

CMS assigns 1–5 star ratings to Medicare and Medicaid-certified nursing homes based on three domains: health inspections, staffing levels, and quality measures. The overall rating is a weighted composite.

A 5-star overall rating does not guarantee excellence in every domain — inspect each sub-rating independently. Data is updated quarterly.
