Staffing Rating
Bear Creek Healthcare LLC holds an overall 4-star rating from CMS, which puts it above average compared to nursing homes across the country. That overall score is built from three separate ratings that each look at something different. The health inspection rating, also 4 stars, reflects that state inspectors found relatively few citations, and nothing too serious, when they reviewed the facility. The staffing rating, likewise 4 stars, tells you that nurses and aides are spending a solid amount of time with each resident compared to what most facilities provide. And the quality measures rating, also 4 stars, is based on 15 clinical indicators like how well the facility manages pain, prevents pressure wounds, and supports residents' physical functioning - all above average here.
When all three pieces come together into that overall 4-star score, it gives you a pretty consistent picture. There are no weak spots dragging things down, which is a good sign. Practically speaking, families can feel reasonably confident that Bear Creek is keeping up with care standards, maintaining adequate staffing levels, and producing solid health outcomes for residents. No facility is perfect, and it is always worth visiting in person and asking questions, but these ratings suggest Bear Creek is a stronger-than-average option worth serious consideration.

Questions to ask when visiting
1. How many residents does each certified nursing assistant care for during the day shift, and does that number change at night or on weekends?
2. How long have most of your nursing staff been working here, and what do you do when you need to fill a shift on short notice?
3. Can you walk me through exactly what happens if a resident falls, has a medication error, or has a medical emergency in the middle of the night?
4. How do you handle a resident who is resistant to care, confused, or showing signs of depression, and who on your team is responsible for that?
5. When was your last state inspection, what did surveyors cite you for, and what specific changes did you make afterward?
6. What does a typical Tuesday look like for a resident who does not have family visiting and does not want to participate in group activities?

About CMS Star Ratings
CMS assigns 1–5 star ratings to Medicare and Medicaid-certified nursing homes based on three domains: health inspections, staffing levels, and quality measures. The overall rating is a weighted composite.
A 5-star overall rating does not guarantee excellence in every domain — inspect each sub-rating independently. Data is updated quarterly.
