Staffing Rating
Gassville Therapy and Living earned an overall 5-star rating from CMS, which means it ranks much above average compared to other nursing homes nationwide. That overall score is a composite of three separate ratings, each looking at a different piece of the picture. The health inspection rating comes in at 4 stars, which is still above average and reflects a relatively clean inspection record with few or minor citations from state surveyors. The staffing rating is a perfect 5 stars, meaning nurses spend significantly more time with residents than you'd see at a typical facility - that kind of hands-on attention can make a real difference in day-to-day care. The quality measures rating is also 5 stars, based on 15 clinical indicators like how well the facility manages pain, prevents infections, and maintains residents' physical function over time.
Practically speaking, this is a strong profile across the board. The one area that isn't a perfect 5 is health inspections, but a 4-star score there is still well above average and shouldn't be a major concern. When all three pieces come together, the overall 5-star rating tells you this facility is consistently performing at a high level in the areas that matter most to families - how often staff is present, how well residents are cared for clinically, and how the facility holds up under regulatory scrutiny.

Questions to ask when visiting
1. How many residents does each certified nursing assistant care for on a typical day shift, and how does that number change at night and on weekends?
2. How long have most of your nursing staff been working here, and what do you do to keep staff from leaving?
3. Can you walk me through exactly what happens if a resident falls or has a medical emergency in the middle of the night?
4. How do you handle it when a resident has dementia or memory issues and becomes upset or agitated - what does your staff actually do in that moment?
5. When was the last time this facility was cited for a deficiency by state inspectors, and can I see the most recent inspection report before I leave today?
6. What does a typical Tuesday look like for a resident who needs help with bathing and meals - who helps them, at what times, and what do they do between those care moments?

About CMS Star Ratings
CMS assigns 1–5 star ratings to Medicare and Medicaid-certified nursing homes based on three domains: health inspections, staffing levels, and quality measures. The overall rating is a weighted composite.
A 5-star overall rating does not guarantee excellence in every domain — inspect each sub-rating independently. Data is updated quarterly.
