Staffing Rating
Manila Healthcare Center holds an overall 5-star rating from CMS, which is about as good as it gets. That top rating is a composite that weighs together three separate scores, and two of them are truly standout. The health inspection rating is 5 stars, meaning state inspectors found very few, if any, serious problems when they visited. That's a strong sign that the facility is being run safely and responsibly on a day-to-day basis. The staffing rating is also 5 stars, which tells you that nurses are spending significantly more time with residents than what you'd see at the average facility. More staff time generally means faster responses, more attentive care, and less chance of residents going unnoticed when something is wrong. The one area worth a closer look is the quality measures rating, which comes in at 2 stars, meaning it's below average. This score is based on 15 clinical and physical health outcomes, things like whether residents are experiencing falls, pressure sores, weight loss, or declines in mobility. A lower score here doesn't automatically mean poor care, but it's a real data point that families shouldn't ignore. It's worth asking the facility directly which specific measures are driving that score and what steps they're taking to improve. The strong inspection and staffing scores suggest the facility has solid fundamentals, but the quality measures gap is a conversation worth having before making a final decision.

Questions to ask when visiting
1. How many residents does each certified nursing assistant care for during a typical day shift, and does that number change at night or on weekends? 2. How long have most of your nursing staff been working here, and what do you do when you need to fill a shift on short notice? 3. Can you walk me through exactly what happens if a resident falls, has a medical emergency, or shows signs that something is off overnight? 4. How does the staff get to know a new resident's routines, preferences, and personality in the first few weeks after move-in? 5. When a resident or family member has a complaint or concern, who do they go to, and can you give me an example of a problem that came up recently and how it was handled? 6. What does a typical weekday look like for a resident who doesn't have many outside visitors or scheduled therapies? About CMS Star Ratings

CMS assigns 1–5 star ratings to Medicare and Medicaid-certified nursing homes based on three domains: health inspections, staffing levels, and quality measures. The overall rating is a weighted composite.

A 5-star overall rating does not guarantee excellence in every domain — inspect each sub-rating independently. Data is updated quarterly.
