Staffing Rating
Lake Forest Senior Living at Mountain Home holds an overall 4-star rating from CMS, which means it performs above average compared to nursing homes nationwide. That overall score is a composite of three separate ratings, each measuring something different about the facility. The health inspection rating, also 4 stars, is encouraging. It tells you that state inspectors found relatively few problems during their visits, and that any issues they did find were not particularly serious. The quality measures rating sits at 3 stars, which is right at the national average. This score reflects how residents are doing on 15 clinical indicators, things like whether residents are experiencing pain, losing mobility, or developing pressure sores. Being average here means outcomes are neither a standout strength nor a red flag.
The piece of the picture that families should pay closest attention to is the staffing rating, which comes in at 2 stars, or below average. This means the facility has fewer nurse hours per resident than most comparable homes. In practical terms, that could mean residents wait longer for assistance, staff feel stretched thin, or there is less time for individualized attention. It does not mean care is unsafe, but it is worth asking the facility directly about their staffing levels, turnover rates, and how they handle nights and weekends. The strong inspection and solid overall score suggest the facility is doing a lot of things right, but the staffing number is a real conversation to have before making a final decision.

Questions to ask when visiting
1. How many residents does each certified nursing assistant care for during a typical day shift and night shift, and has that number changed in the past year?
2. If my loved one has a concern or feels unsafe, who do they talk to, and can you walk me through exactly what happens after they report it?
3. How long have your charge nurses and CNAs been working here, and what is your average staff turnover rate over the past 12 months?
4. What does a typical Tuesday look like for a resident who does not have scheduled therapy or medical appointments that day?
5. Can you show me the most recent state inspection report and explain any deficiencies that were cited and what was done to fix them?
6. If my loved one's condition changes suddenly in the middle of the night, who makes the call to contact our family, how quickly does that happen, and who is the backup if that person is not available?

About CMS Star Ratings
CMS assigns 1–5 star ratings to Medicare and Medicaid-certified nursing homes based on three domains: health inspections, staffing levels, and quality measures. The overall rating is a weighted composite.
A 5-star overall rating does not guarantee excellence in every domain — inspect each sub-rating independently. Data is updated quarterly.
