Staffing Rating
The Springs of Mine Creek holds an overall 5-star rating from CMS, which is the highest possible score. That overall rating is built from three separate pieces: health inspections, staffing levels, and quality measures. When most of those pieces are strong, the composite score reflects that strength, even if one area needs attention. Looking at the individual pieces, the health inspection rating of 5 stars is excellent news. It means state inspectors have found very few, if any, serious problems during their visits, which is a strong signal that the facility is well-run day to day. The quality measures rating is also 5 stars, meaning residents at this facility tend to fare very well across 15 tracked health outcomes, things like falls, pressure wounds, and hospital readmissions. The one area worth keeping in mind is the staffing rating of 2 stars, which falls below average. This reflects that nurses spend fewer hours per resident compared to similar facilities, which can sometimes mean staff are stretched thin. The good news is that the clinical outcomes are still excellent despite this, suggesting the team may be working efficiently, but it is a fair question to ask the facility about when you visit.

Questions to ask when visiting
1. How many residents does each certified nursing assistant care for during the day shift, and does that number change at night or on weekends? 2. How long have most of your nurses and aides been working here, and what does your staff turnover look like over the past year? 3. If my loved one has a concern or feels unsafe, what exactly happens after they report it, and can you walk me through a real example of how that was handled? 4. What does a typical weekday look like for a resident who does not have many scheduled therapies, and how much time do they spend outside their room? 5. How many falls happened in this facility over the last six months, and what changes were made after those incidents? 6. Can I see the most recent state inspection report, and can someone here explain the deficiencies that were cited and what was done to correct them? About CMS Star Ratings

CMS assigns 1–5 star ratings to Medicare and Medicaid-certified nursing homes based on three domains: health inspections, staffing levels, and quality measures. The overall rating is a weighted composite.

A 5-star overall rating does not guarantee excellence in every domain — inspect each sub-rating independently. Data is updated quarterly.
