Staffing Rating
Southfork River Therapy and Living earned an overall 5-star rating from CMS, which is the highest possible score. That top-level rating is a composite that weighs three separate areas together, so it's worth understanding what each one tells you. The health inspection rating is a perfect 5 stars, meaning state inspectors found very few, if any, serious concerns during their visits. That's a strong signal that the facility is well-managed and consistently meeting care standards. The quality measures rating comes in at 4 stars, which is above average across 15 clinical indicators like fall rates, pressure ulcers, and how well residents maintain their physical functioning. That suggests residents are generally experiencing good health outcomes day to day. The one area to be aware of is staffing, which sits at 3 stars, meaning average. This reflects the number of nursing hours provided per resident compared to other facilities nationwide. It doesn't necessarily mean care is lacking, but it does mean families may want to ask questions during a tour about how many nurses and aides are on the floor at different times of day, especially overnight. Keep in mind that a 5-star overall rating despite average staffing tells you the facility is performing well in ways the other categories measure, so this is more of a conversation starter than a red flag.

Questions to ask when visiting
1. How many residents does each certified nursing assistant care for during the day shift, and does that number change at night or on weekends? 2. How long have your charge nurses and CNAs been working here on average, and what does your turnover look like over the past year? 3. If my loved one has a concern or feels unsafe, who do they tell, and what happens after that - can you walk me through a real example of how a complaint was handled? 4. How do you handle a resident who starts declining mentally or physically - who notices the change first, and how quickly does a care plan get updated? 5. What does a typical weekday actually look like for a resident who isn't in therapy - when do they eat, what activities are available, and how much time do they spend outside their room? 6. Can I see your most recent state inspection report, and can someone here explain what was cited and what was done to fix it? About CMS Star Ratings

CMS assigns 1–5 star ratings to Medicare and Medicaid-certified nursing homes based on three domains: health inspections, staffing levels, and quality measures. The overall rating is a weighted composite.

A 5-star overall rating does not guarantee excellence in every domain — inspect each sub-rating independently. Data is updated quarterly.
