Staffing Rating
Abbey Delray South earns 4 out of 5 stars overall, which means it performs above average compared to other nursing homes nationwide. That overall number is a composite drawn from three separate ratings, and in this case, two of them are standouts. The staffing rating is a perfect 5 stars, meaning residents here receive significantly more nurse time per day than the average facility. The quality measures rating is also 5 stars, reflecting strong outcomes across 15 clinical indicators like fall rates, wound care, and pain management. Simply put, residents tend to be well cared for and the staff has the time to provide that care.
The one area worth discussing with the facility is the health inspection rating, which sits at 2 stars, meaning inspectors found more citations or more serious ones than you would see at a typical nursing home. That does pull the overall score down from what would otherwise be an exceptional profile. Health inspections look at things like safety protocols, documentation, and regulatory compliance during state visits. It does not necessarily mean residents are unhappy or unsafe day to day, especially given the strong staffing and outcomes numbers, but it is worth asking the facility directly what those citations were for and what steps they have taken to address them.

Questions to ask when visiting
1. How many residents does each certified nursing assistant typically care for during the day shift, and does that number change at night or on weekends?
2. How often do residents see the same aides and nurses week to week, and what is your current staff turnover rate?
3. Can you walk me through exactly what happens if a resident falls, has a medical emergency, or shows signs of a sudden health change in the middle of the night?
4. How do you handle a resident who refuses a meal, misses activities for several days in a row, or seems withdrawn, and who specifically follows up on that?
5. What does a typical weekday look like for a resident who needs help with bathing, dressing, and getting to meals, and how much of that schedule is flexible around what the resident prefers?
6. Has this facility been cited for any deficiencies in the last two state inspections, and can I see the most recent survey report before I leave today?

About CMS Star Ratings
CMS assigns 1–5 star ratings to Medicare and Medicaid-certified nursing homes based on three domains: health inspections, staffing levels, and quality measures. The overall rating is a weighted composite.
A 5-star overall rating does not guarantee excellence in every domain — inspect each sub-rating independently. Data is updated quarterly.
