Staffing Rating
Lakeside Center For Rehabilitation And Healing holds a 5-star overall rating from CMS, which is the highest possible score. That overall number is a composite of three separate ratings, and it's worth understanding what each one tells you. The 4-star health inspection rating means state inspectors found fewer and less serious violations than most facilities - a reassuring sign that the building is being run safely and that problems, when they do come up, tend to be minor. The 5-star quality measures rating is the standout here - it reflects how residents are actually doing physically and clinically across 15 different health outcomes, things like falls, pressure wounds, and how well residents maintain their ability to function. Scoring at the very top of the scale there is a meaningful indicator of strong day-to-day care. The one area to pay attention to is staffing, which comes in at 2 stars, meaning nurses are spending less time with each resident than you'd see at most comparable facilities. That's worth asking the facility about directly.
The overall 5-star rating is impressive and speaks well of Lakeside Center in most respects, but no single number tells the whole story. The gap between the excellent quality outcomes and the below-average staffing is something families should think about - strong outcomes despite leaner staffing could mean the team is very efficient, but it could also mean staff are stretched thin in ways that aren't always visible in outcome data. When you visit, it's reasonable to ask how many residents each nurse or aide is typically responsible for during a shift.

Questions to ask when visiting
1. How many residents does each certified nursing aide care for during the day shift, and does that ratio change at night or on weekends?
2. How long have your charge nurses and CNAs been working here, and what does your staff turnover look like over the past year?
3. If my family member fell or had a medical emergency in the middle of the night, walk me through exactly what would happen from the moment it occurred until we were notified.
4. When a resident doesn't want to get up, skip a meal, or refuses a bath, how does your staff handle that - and who makes the call on how to respond?
5. Can I see the last two state inspection reports, and can you tell me what happened after any deficiencies were cited?
6. What does a typical Tuesday look like for a resident who doesn't have any scheduled therapy - how do they spend their morning, afternoon, and evening?

About CMS Star Ratings
CMS assigns 1–5 star ratings to Medicare and Medicaid-certified nursing homes based on three domains: health inspections, staffing levels, and quality measures. The overall rating is a weighted composite.
A 5-star overall rating does not guarantee excellence in every domain — inspect each sub-rating independently. Data is updated quarterly.
