Staffing Rating
Majestic Oaks holds a 5-star overall rating from CMS, which is about as strong as it gets. That overall score is built from three separate ratings, and Majestic Oaks scores at the top in two of them. The health inspection rating of 5 stars means state inspectors found very few problems during their visits, and any issues they did find were minor. That's a meaningful signal for families, since inspections look closely at whether a facility is following safe, proper care practices. The staffing rating is also 5 stars, which tells you nurses and aides are spending more time with residents than at most other facilities. More staff time generally means faster response to calls, more attentive daily care, and less risk of residents going unnoticed when something is wrong. The quality measures rating comes in at 4 stars, which is still above average. This score is based on 15 clinical and physical health outcomes for residents, things like whether residents are experiencing falls, pressure wounds, or declines in mobility. A 4 here means Majestic Oaks is performing better than most facilities, even if it hasn't quite reached the top tier on every outcome measure. Taken together, these three pieces paint a very reassuring picture. Strong inspections, strong staffing, and solid health outcomes all pointing in the same direction is not something families should take for granted when comparing facilities.

Questions to ask when visiting
1. How many residents does each certified nursing assistant typically care for during the day shift, and does that number change at night or on weekends? 2. How long have your charge nurses and CNAs been working here on average, and what does your staff turnover look like compared to last year? 3. Can you walk me through exactly what happens when a resident falls or has a medical emergency, from the moment it occurs to when the family is notified? 4. If my loved one needs help getting up, bathing, or eating a meal, how do you make sure they are not waiting a long time for that help? 5. What does a typical weekday look like for residents who are able to leave their room, and what activities or social time are available for those who mostly stay in bed? 6. Has this facility received any citations or fines from the state in the last two years, and if so, what specifically changed to make sure the same problem does not happen again? About CMS Star Ratings

CMS assigns 1–5 star ratings to Medicare and Medicaid-certified nursing homes based on three domains: health inspections, staffing levels, and quality measures. The overall rating is a weighted composite.

A 5-star overall rating does not guarantee excellence in every domain — inspect each sub-rating independently. Data is updated quarterly.
