Staffing Rating
Solaris Healthcare Windermere holds an overall 5-star rating from CMS, which is about as strong as it gets. That top-level score is a composite picture drawn from three separate categories, and this facility scores exceptionally well across all of them. The health inspection rating - also 5 stars - reflects that state inspectors found very few, if any, serious problems during their visits. Fewer citations, and less severe ones, generally means the facility is being run carefully and that residents aren't falling through the cracks on safety or care standards. The quality measures rating comes in at 5 stars as well, meaning that across 15 clinical indicators - things like how often residents experience falls, pressure wounds, or declines in daily functioning - this facility is performing much better than most. That's a meaningful signal that residents are getting hands-on care that's actually making a difference in their health outcomes. The one area that steps back slightly is staffing, which earns 4 stars — still above average, but not at the top tier. This rating reflects how many nursing hours residents receive relative to the number of people living there. A 4 here means families can feel good about staffing levels, though it's always worth asking during a tour how the facility handles nights, weekends, and busy periods. Overall, though, this is a facility that checks nearly every box, and the combination of strong inspections, solid staffing, and excellent clinical outcomes gives families a well-rounded reason for confidence.

Questions to ask when visiting
1. What is your current staff-to-resident ratio during the day shift, and how does that change at night and on weekends? 2. How long have most of your certified nursing assistants worked here, and what do you do to keep staff from leaving? 3. If my loved one had a fall or a medical emergency overnight, walk me through exactly what would happen - who gets called, in what order, and how quickly? 4. Can you show me the most recent state inspection report, and can you explain any deficiencies that were cited? 5. What does a typical Tuesday look like for a resident - what time do they wake up, eat, and what activities are actually available to them that day? 6. If my family member is unhappy here or feels their concerns aren't being heard, what is the process for raising a complaint, and who outside this facility can we contact? About CMS Star Ratings

CMS assigns 1–5 star ratings to Medicare and Medicaid-certified nursing homes based on three domains: health inspections, staffing levels, and quality measures. The overall rating is a weighted composite.

A 5-star overall rating does not guarantee excellence in every domain — inspect each sub-rating independently. Data is updated quarterly.
