Staffing Rating
Chatsworth At PGA National holds a 5-star overall rating from CMS, which is about as strong as it gets. That top-level rating is built from three pieces: health inspections, staffing, and quality measures. When those components consistently score well, the overall rating reflects a facility that is performing at a high level across the board.
On the inspection side, the 5-star health inspection rating tells you that state surveyors found very few problems, and nothing serious, during their visits. That is a meaningful signal that the facility is being run carefully and that residents are living in a safe, well-maintained environment. The 5-star staffing rating means nurses and aides are spending significantly more time with each resident than you would typically see elsewhere, which often translates to faster responses, better care, and less reliance on stretched-thin staff. The quality measures rating comes in at 4 stars, which is still above average. This score is based on 15 clinical indicators, things like how residents are managing pain, whether they are experiencing falls or pressure sores, and other concrete health outcomes. A 4 here means the facility is doing better than most, even if there is a little room to improve on some of those specific measures. Taken together, this is a reassuring profile for families weighing their options.

Questions to ask when visiting
1. How many residents does each certified nursing assistant care for during the day shift, and does that number change at night or on weekends?
2. How long have most of your direct care staff been working here, and what is your turnover rate over the past year?
3. If my family member had a fall or a medical emergency overnight, walk me through exactly what would happen and who would be notified.
4. Can I come back for an unannounced visit at a different time of day, including evenings or weekends, to see how things run?
5. How do you handle a resident who refuses care or becomes upset, and who on your staff is trained to manage those situations?
6. What does a typical weekday look like for a resident who does not have many visitors, from the time they wake up until they go to bed?

About CMS Star Ratings
CMS assigns 1–5 star ratings to Medicare and Medicaid-certified nursing homes based on three domains: health inspections, staffing levels, and quality measures. The overall rating is a weighted composite.
A 5-star overall rating does not guarantee excellence in every domain — inspect each sub-rating independently. Data is updated quarterly.
