Staffing Rating
Pavilion For Health Care, The earns a perfect 5-star overall rating from CMS, which is built from three separate scored areas that each tell you something different about how a facility operates. The health inspection rating reflects how the facility performed during state inspections, and a 5 here means inspectors found very few, if any, serious problems, which is a strong sign that day-to-day care and safety standards are being upheld consistently. The staffing rating shows that nurses and aides are spending significantly more time with residents compared to most other facilities, meaning your loved one is less likely to wait long for help or feel overlooked. The quality measures rating is based on 15 clinical indicators, things like whether residents are developing pressure sores, losing too much weight, or experiencing falls, and a 5 here tells you the facility is achieving very good health outcomes for the people living there.
When all three of those scores are combined, you get the overall rating, and a 5 across the board is genuinely rare. It means this facility is not just strong in one area while making up for weaknesses in another. It is performing at a high level in compliance, in how it deploys its staff, and in the actual physical wellbeing of its residents. For a family trying to make one of the hardest decisions they will face, that kind of consistency is a very reassuring starting point.

4. Can you walk me through what a typical weekday looks like for a resident, from morning until they go to sleep at night?
5. How do you handle situations where a resident's condition changes quickly, and how soon would you contact our family?
6. What was your most recent state inspection score, and can I see the report or find out where to look it up on my own?

About CMS Star Ratings
CMS assigns 1–5 star ratings to Medicare and Medicaid-certified nursing homes based on three domains: health inspections, staffing levels, and quality measures. The overall rating is a weighted composite.
A 5-star overall rating does not guarantee excellence in every domain — inspect each sub-rating independently. Data is updated quarterly.
