Staffing Rating
Arcadia Health and Rehabilitation Center holds a 4-star overall rating from CMS, which places it above average compared to nursing homes nationwide. That overall score is a composite built from three separate ratings, so it helps to look at each piece on its own. The health inspection rating comes in at 4 stars, meaning state inspectors found fewer or less serious issues here than at most facilities - a good sign that the home is being run safely and responsibly. Staffing also earns 4 stars, which tells you that nurses and aides are spending more time with residents than you'd typically see elsewhere, and that tends to translate into more attentive, responsive day-to-day care. The one area worth paying closer attention to is quality measures, which scores 3 stars, landing right at average. This rating is based on 15 clinical indicators tracking things like how residents fare with pain management, pressure ulcers, falls, and other health outcomes over time. Average isn't alarming, but it does suggest there may be room for improvement in some of those concrete health results. When you visit or call the facility, it would be reasonable to ask staff which specific measures they are actively working to improve. Overall, the 4-star composite reflects a facility that is performing well in most areas, with quality outcomes being the one category where families should ask a few extra questions.

Questions to ask when visiting
1. How many residents does each certified nursing assistant care for during the day shift, and does that number change at night or on weekends? 2. How long have your nursing staff and CNAs been working here on average, and what does your turnover look like over the past year? 3. If my loved one falls or has a medical emergency overnight, walk me through exactly what happens from the moment it's discovered to when the family is contacted. 4. How do you handle a resident who refuses a bath, a meal, or their medication, and can you give me a real example of how staff worked through that recently? 5. What does a typical Tuesday look like for a resident who isn't mobile, from the time they wake up to the time they go to sleep? 6. Has this facility been cited for any deficiencies in the last two state inspections, and if so, what specific changes did you make as a result? About CMS Star Ratings

CMS assigns 1–5 star ratings to Medicare and Medicaid-certified nursing homes based on three domains: health inspections, staffing levels, and quality measures. The overall rating is a weighted composite.

A 5-star overall rating does not guarantee excellence in every domain — inspect each sub-rating independently. Data is updated quarterly.
