Staffing Rating
Port St. Lucie Rehabilitation and Healthcare holds an overall 4-star rating from CMS, which means it performs above average compared to nursing homes nationwide. That overall score is a composite built from three separate ratings, each looking at a different piece of what the facility actually delivers. The health inspection rating comes in at 4 stars, reflecting that state inspectors found relatively few citations and nothing severe enough to drag the score down, which is a good sign that the facility is meeting care and safety standards. The staffing rating is also 4 stars, meaning nurses and aides are spending more time with residents than you'd typically find at comparable facilities, and more staff time generally translates to more attentive, responsive care on a day-to-day basis. The one area worth paying attention to is the quality measures rating, which sits at 2 stars. This score is based on 15 clinical and physical health outcomes, things like whether residents are experiencing pressure sores, losing too much weight, or seeing their physical abilities decline over time. A 2-star score here means those outcomes are running below average, and that gap is worth discussing directly with the facility. It does not cancel out the stronger inspection and staffing scores, but it does suggest families should ask questions about how the facility tracks and improves resident health over the course of a stay.

Questions to ask when visiting
1. How many residents is each certified nursing assistant responsible for during a typical day shift, and does that number change at night or on weekends? 2. How long have your charge nurses and CNAs been working here on average, and what does your staff turnover look like over the past year? 3. If my loved one had a fall or a sudden change in condition overnight, walk me through exactly what happens, who gets called, and how quickly? 4. Can I see the most recent state inspection report, and can you explain any deficiencies listed and what you did to fix them? 5. What does a typical weekday actually look like for a resident, from when they wake up to when they go to bed, including meals, activities, and time outside their room? 6. How do you handle a situation where a family member has a concern or complaint about the care their loved one is receiving, and who specifically would I talk to? About CMS Star Ratings

CMS assigns 1–5 star ratings to Medicare and Medicaid-certified nursing homes based on three domains: health inspections, staffing levels, and quality measures. The overall rating is a weighted composite.

A 5-star overall rating does not guarantee excellence in every domain — inspect each sub-rating independently. Data is updated quarterly.
