Staffing Rating
Ponce Therapy Care Center And Rehab, The holds an overall 5-star rating from CMS, which is the highest possible score. That top mark is driven largely by a perfect 5-star Health Inspection rating, meaning state inspectors found very few, if any, serious problems during their visits. That's a meaningful signal for families because it reflects how the facility operates day to day when regulators are watching, and when they're not. The Quality Measures rating comes in at 4 stars, which is above average across the 15 clinical indicators CMS tracks, things like how well the facility manages pain, prevents pressure sores, and supports residents' physical functioning. That tells you the care residents actually receive tends to produce better-than-average health outcomes. The one area worth keeping in mind is the 2-star Staffing rating, which falls below average. This reflects that the facility has fewer nurse hours per resident compared to most nursing homes nationally. Even at a highly-rated facility, lower staffing numbers can mean residents sometimes wait longer for help or have less one-on-one time with nurses. It does not automatically mean care is poor, especially given the strong inspection and quality scores, but it's a fair question to raise when you visit. Ask about how they handle nights and weekends, and how staff communicate with families. CMS combines all three areas into the overall rating, and here the strength of the inspection and quality results is enough to carry the composite to 5 stars despite the staffing gap.

Questions to ask when visiting
1. How many residents does each certified nursing assistant care for during a typical day shift, and does that ratio change at night or on weekends? 2. How long have most of your nursing staff been working here, and what does your turnover look like over the past year? 3. If my loved one has a fall, a medication error, or a sudden health change, what exactly happens next and how quickly will you contact me? 4. Can you walk me through what a typical day looks like for a resident who needs help with meals, bathing, and getting around? 5. How do you handle complaints from residents or family members, and can you give me an example of a problem that came up recently and how it was resolved? 6. Has this facility been cited for any deficiencies in the last state inspection, and where can I read the full report? About CMS Star Ratings

CMS assigns 1–5 star ratings to Medicare and Medicaid-certified nursing homes based on three domains: health inspections, staffing levels, and quality measures. The overall rating is a weighted composite.

A 5-star overall rating does not guarantee excellence in every domain — inspect each sub-rating independently. Data is updated quarterly.
