Staffing Rating
Terrace of St. Cloud holds an overall 4-star rating from CMS, which means it performs above average compared to nursing homes nationally. That overall score is a composite of three separate sub-ratings, and it's worth looking at each one individually because they tell different stories. The health inspection rating is also 4 stars, which is a strong sign. This reflects the number and severity of citations found during state inspections, so a high score here suggests the facility has generally been meeting care and safety standards when regulators come through. The quality measures rating sits at 3 stars, landing right at average. This area tracks 15 clinical outcomes for residents, things like how often residents experience falls, pressure sores, or other physical health changes, so average here means the facility is keeping pace with peers but not standing out.
The one area families should pay attention to is the staffing rating, which comes in at 2 stars, meaning it falls below average. This reflects how many hours of nursing care residents receive relative to the number of people living there. Lower staffing numbers can sometimes mean residents wait longer for assistance or get less one-on-one attention from nurses and aides. It does not automatically mean care is poor, but it is worth asking the facility directly about their staffing levels and how they manage coverage, especially during nights and weekends. Taken together, the strong inspection history is reassuring, but the staffing score is a reasonable conversation starter before you make a final decision.

Questions to ask when visiting
1. How many residents does each certified nursing assistant care for during the day shift, and does that number change at night or on weekends?
2. How long have most of your direct care staff been working here, and what is your annual turnover rate for CNAs and nurses?
3. If my family member has a concern or feels unsafe, who do they go to, and how quickly will someone actually respond?
4. Can you walk me through what a typical weekday looks like for a resident who needs help with bathing, meals, and getting around?
5. How do you handle a situation where a resident's condition changes suddenly, and how quickly does the family get notified?
6. Has this facility been cited for any deficiencies in the past two years, and if so, what specifically changed to make sure it does not happen again?

About CMS Star Ratings
CMS assigns 1–5 star ratings to Medicare and Medicaid-certified nursing homes based on three domains: health inspections, staffing levels, and quality measures. The overall rating is a weighted composite.
A 5-star overall rating does not guarantee excellence in every domain — inspect each sub-rating independently. Data is updated quarterly.
