Staffing Rating
Benderson Family Skilled Nursing And Rehab Center holds an overall 5-star rating from CMS - the highest possible - which is a composite of three separate scores that each look at a different piece of what daily life and care look like inside a facility. The health inspection rating is a perfect 5 stars, meaning state inspectors found very few, if any, serious problems during their visits. That's a strong sign that the facility is being run carefully and that residents aren't being put at risk through neglect or poor practices. The staffing rating is also 5 stars, which means nurses and aides are spending significantly more time with each resident than you'd typically see elsewhere. In practical terms, that translates to call lights getting answered, residents being turned and repositioned to prevent bedsores, and staff having enough bandwidth to actually know their residents. The quality measures rating comes in at 4 stars - above average, though not perfect. This score is drawn from 15 clinical indicators, things like how often residents develop infections, experience falls, lose too much weight, or need to be hospitalized. A 4 here is genuinely good and means outcomes for residents are better than most facilities, even if there's a small margin of room to improve. Taken together, these three scores paint a picture of a facility that's well-staffed, well-managed, and delivering strong results for the people in its care - which is about as reassuring a combination as a family can find when weighing options.

Questions to ask when visiting
1. How many residents does each certified nursing assistant care for during the day shift, and how does that change at night and on weekends? 2. How long have your charge nurses and CNAs been working here on average, and what does your staff turnover look like over the past year? 3. If my loved one had a fall or a sudden change in condition overnight, walk me through exactly what happens - who gets called, in what order, and how quickly? 4. What does a typical Tuesday look like for a resident who doesn't have therapy scheduled — how do they spend their morning, afternoon, and evening? 5. Can you show me the most recent state inspection report, and can you explain what was cited and what's been done to fix it? 6. If my family member isn't eating well or seems withdrawn, how would your team catch that, and who would reach out to us and when? About CMS Star Ratings

CMS assigns 1–5 star ratings to Medicare and Medicaid-certified nursing homes based on three domains: health inspections, staffing levels, and quality measures. The overall rating is a weighted composite.

A 5-star overall rating does not guarantee excellence in every domain — inspect each sub-rating independently. Data is updated quarterly.
