Staffing Rating
Lakeside Health Center holds a 4-star overall rating from CMS, which puts it above average compared to nursing homes across the country. That overall score is built from three separate ratings that each measure something different, so it helps to look at what each one is actually telling you.
The 4-star Health Inspection rating means that when state inspectors came through, they found fewer or less serious problems than most facilities see. That's a good sign for day-to-day safety and care standards. The 4-star Staffing rating tells you that nurses are spending more time with residents than average, which generally translates to more attentive, responsive care. The one area to keep in mind is the 3-star Quality Measures rating, which lands at average. This score is based on 15 clinical indicators, things like how residents are doing physically over time, rates of falls, pressure ulcers, and similar health outcomes. It's not a red flag, but it does suggest there may be some room for improvement in long-term resident health results. Taken together, Lakeside is a solid above-average choice, with particular strengths in inspection performance and staffing levels.

Questions to ask when visiting
1. How many residents does each certified nursing assistant typically care for during the day shift, and how does that number change at night and on weekends?
2. How long have your nurses and aides been working here on average, and what is your staff turnover rate over the past year?
3. Can you walk me through exactly what happens if a resident falls, has a medical emergency, or shows signs of a sudden change in health, from the moment it happens to when the family is notified?
4. What does a typical weekday look like for a resident who needs memory care, and how often are they actually out of their room and engaging with others?
5. How many deficiencies or citations has this facility received in the last two state inspection reports, and can I see those reports?
6. If my loved one has a complaint or feels they are being ignored or mistreated, what is the specific process for reporting that, and who outside of this facility can they contact?

About CMS Star Ratings
CMS assigns 1–5 star ratings to Medicare and Medicaid-certified nursing homes based on three domains: health inspections, staffing levels, and quality measures. The overall rating is a weighted composite.
A 5-star overall rating does not guarantee excellence in every domain — inspect each sub-rating independently. Data is updated quarterly.
