Staffing Rating
Schervier Nursing Care Center holds a 4-out-of-5-star overall rating from CMS, which means it lands above average compared to nursing homes across the country. That overall number is a composite of three separate scores, and it's worth looking at each one individually since they tell different stories. The Health Inspection rating sits at 3 stars, meaning the facility is about average when it comes to the number and seriousness of citations found during state inspections - not a red flag, but not a standout either. The Staffing rating is 2 stars, which is below average and signals that nurses and aides are carrying more residents per staff member than you'd ideally want to see. That can translate to longer wait times for assistance and less one-on-one attention for your loved one on a day-to-day basis. On the bright side, the Quality Measures rating is a perfect 5 stars, meaning resident health outcomes - things like how well pain is managed, whether residents are maintaining mobility, and how rates of infections or falls compare to other facilities - are among the best in the country.
The strong Quality Measures score clearly does a lot of the heavy lifting in pulling that overall rating up to 4 stars. Practically speaking, families should feel encouraged by the outcomes data, since it suggests residents are genuinely being well cared for in terms of their physical health. At the same time, the lower staffing number is worth asking about directly when you visit. Finding out how the facility manages scheduling, and what the typical nurse-to-resident ratio looks like on nights and weekends, would be a smart conversation to have before making a final decision.

Questions to ask when visiting
1. How many residents does each certified nursing assistant care for during the day shift, and does that number change at night or on weekends?
2. How long have your charge nurses and CNAs been working here on average, and what does your staff turnover look like over the past year?
3. If my loved one falls or has a medical emergency in the middle of the night, walk me through exactly what happens from the moment staff are alerted to when the family is notified.
4. How does the facility handle residents who wander or show signs of memory loss, and what physical safeguards are in place to keep them safe?
5. Can I come back unannounced at different times of day to visit, and are there any restrictions on when family members can be here?
6. What does a typical Tuesday look like for a resident who needs help with most daily activities, from the time they wake up to the time they go to bed?

About CMS Star Ratings
CMS assigns 1–5 star ratings to Medicare and Medicaid-certified nursing homes based on three domains: health inspections, staffing levels, and quality measures. The overall rating is a weighted composite.
A 5-star overall rating does not guarantee excellence in every domain — inspect each sub-rating independently. Data is updated quarterly.
