Staffing Rating
The Heritage Rehabilitation and Health Care Center holds a 5-star overall rating from CMS, which is the highest possible score. That overall number is a composite of three separate ratings, and it's worth understanding what each one tells you. The Health Inspection rating is also a perfect 5 stars, meaning state inspectors found very few or no significant problems during their visits. That's a strong sign that the facility is being run safely and is holding up well under outside scrutiny. The Quality Measures rating sits at 3 stars, which is average. This score reflects clinical outcomes across 15 different health indicators, things like how residents are managing pain, whether they're experiencing falls, or how wounds are healing. Average here means the facility is performing in line with most nursing homes, not standing out in either direction on resident health outcomes.
The one area worth paying close attention to is the Staffing rating, which comes in at 2 stars, meaning below average. This reflects how many hours of nursing care residents are receiving relative to the number of people living there. Lower staffing levels can sometimes mean residents wait longer for help, get less one-on-one attention, or feel rushed during daily care routines. It doesn't necessarily mean care is unsafe, but it's a practical concern worth raising directly with the facility. When you visit, ask about nurse-to-resident ratios on different shifts, especially evenings and weekends. The strong inspection score helps explain how the overall rating climbs to 5 stars despite this lower staffing number, but staffing is something families should factor into their own decision.

Questions to ask when visiting
1. How many residents does each certified nursing assistant typically care for during a day shift, and does that number change at night or on weekends?
2. How long have most of your nursing staff been working here, and what do you do to keep turnover low?
3. Can you walk me through exactly what happens if a resident falls or has a medical emergency in the middle of the night?
4. How do you handle a resident who refuses a meal, misses medications, or seems more withdrawn than usual?
5. What does a typical weekday look like for a resident who doesn't have many visitors or scheduled activities?
6. If I have a concern about my loved one's care, who do I call, how quickly will someone respond, and what happens if the problem isn't resolved?

About CMS Star Ratings
CMS assigns 1–5 star ratings to Medicare and Medicaid-certified nursing homes based on three domains: health inspections, staffing levels, and quality measures. The overall rating is a weighted composite.
A 5-star overall rating does not guarantee excellence in every domain — inspect each sub-rating independently. Data is updated quarterly.
