Staffing Rating
Fairview Nursing Care Center Inc. holds a 4-star overall rating, which puts it above average compared to nursing homes across the country. That overall score is a composite that pulls together three separate areas, and in Fairview's case, one of those areas is pulling the total up significantly. The Quality Measures rating is a perfect 5 stars, meaning the facility performs much better than average on 15 clinical and physical health outcomes tracked for residents, things like how well wounds are managed, whether residents are experiencing falls, and how their overall physical condition holds up over time. That's a meaningful signal that the day-to-day care residents actually receive is producing strong results. The other two ratings land at 3 stars each, which puts them right at average. The Health Inspection score reflects findings from state inspections, and a 3 here means Fairview has had a typical number and severity of citations, not a red flag, but not a standout record either. The Staffing score also sits at average, meaning the number of nursing hours residents receive relative to the size of the resident population is in line with most facilities, though not exceptional. For families, the takeaway is that Fairview isn't a perfect facility across the board, but its strong clinical outcomes suggest that despite average staffing levels, the care being delivered is genuinely effective. It's a place worth a closer look, particularly if health outcomes are your top priority.

Questions to ask when visiting
1. How many residents does each certified nursing assistant care for during a typical day shift, and does that number change on nights and weekends? 2. How long have your charge nurses and CNAs been working here on average, and what does your staff turnover look like over the past year? 3. If my loved one had a fall or a sudden change in condition overnight, walk me through exactly what happens - who gets called, in what order, and how quickly would I hear from someone? 4. Can you show me the most recent state inspection report, and can you explain what was cited and what has been done to fix it? 5. What does a typical Tuesday look like for a resident who does not have family visiting - what activities happen, how much time do they spend outside their room, and who checks in on them throughout the day? 6. If my loved one has a concern or a complaint, what is the process for raising it, who handles it, and how would I know it was actually addressed? About CMS Star Ratings

CMS assigns 1–5 star ratings to Medicare and Medicaid-certified nursing homes based on three domains: health inspections, staffing levels, and quality measures. The overall rating is a weighted composite.

A 5-star overall rating does not guarantee excellence in every domain — inspect each sub-rating independently. Data is updated quarterly.
