Staffing Rating
Greenfield Health & Rehab Center earns a 5-star overall rating from CMS, which is the highest possible score and reflects strong performance across most areas. The overall rating is a composite that weighs all three sub-ratings together, so a facility can score at the top even if one area lands closer to average, as long as the others are strong enough to carry it. Looking at the individual pieces, the 5-star staffing rating is a standout, meaning residents here receive significantly more nurse time per day than you would see at a typical facility. That matters practically because more staffing generally means faster responses, better personal care, and less likelihood of things falling through the cracks. The 4-star health inspection rating tells you that state surveyors found very few problems during their visits, and any citations that did come up were minor rather than serious. The one area to keep in mind is the 3-star quality measures rating, which sits right at average. Those scores are based on 15 clinical outcomes like fall rates, pressure ulcers, and hospital readmissions, so it is worth asking the facility directly about how they track and work to improve in those areas. Average is not alarming, but when you are comparing facilities, it is a useful conversation starter.

Questions to ask when visiting
1. How many residents does each certified nursing assistant typically care for during the day shift, and does that number change at night or on weekends? 2. How long have most of your nursing staff been working here, and what do you do when a scheduled nurse or aide calls out sick? 3. Can you walk me through exactly what happens if a resident falls or has a medical emergency in the middle of the night? 4. How do you handle a resident who resists care, gets confused, or becomes upset, and do you use any medications to manage behavior? 5. What does a typical Tuesday look like for a resident who is mobile and social, versus one who stays mostly in their room? 6. If I have a concern about my loved one's care, who do I call, how quickly can I expect a response, and what happens if the problem is not resolved? About CMS Star Ratings

CMS assigns 1–5 star ratings to Medicare and Medicaid-certified nursing homes based on three domains: health inspections, staffing levels, and quality measures. The overall rating is a weighted composite.

A 5-star overall rating does not guarantee excellence in every domain — inspect each sub-rating independently. Data is updated quarterly.
