Staffing Rating
Conesus Lake Nursing Home, LLC holds an overall 4-star rating from CMS, which means it performs above average compared to nursing homes nationwide. That overall score is built from three separate ratings, each measuring something different, and all three come in at 4 stars here, which tells a fairly consistent story about the facility. The Health Inspection rating reflects how the facility did during state inspections, looking at how many issues were found and how serious they were. A 4-star score here means inspectors found fewer or less serious problems than most facilities, which is a good sign for families concerned about safety and care standards day to day. The Staffing rating is based on how many hours of nursing care residents receive relative to the number of people living there, and above average means residents here generally have more access to nurses and aides than at a typical facility. The Quality Measures rating tracks 15 clinical indicators, things like how often residents experience falls, pressure sores, or declines in mobility, and a 4-star score suggests residents at this facility tend to fare better on those health outcomes than average. Taken together, these ratings point to a facility that is performing well across the board, not just in one area.

Questions to ask when visiting
1. How many residents does each certified nursing assistant typically care for during a day shift, and does that number change on nights and weekends? 2. How long have your charge nurses and CNAs been working here on average, and what does your staff turnover look like over the past year? 3. If my family member falls or has a medical emergency at 2 in the morning, walk me through exactly what happens from the moment staff finds out. 4. How do you handle a resident who refuses a bath, skips meals, or pushes back on their care routine - what does that actually look like day to day? 5. Can you show me the most recent state inspection report, and can you explain any deficiencies that were cited and what you did to fix them? 6. If I want to visit on a Tuesday evening or a Saturday morning without calling ahead, is that allowed, and is there ever a time when family cannot come in? About CMS Star Ratings

CMS assigns 1–5 star ratings to Medicare and Medicaid-certified nursing homes based on three domains: health inspections, staffing levels, and quality measures. The overall rating is a weighted composite.

A 5-star overall rating does not guarantee excellence in every domain — inspect each sub-rating independently. Data is updated quarterly.
