Staffing Rating
NYS Veterans Home earns an overall 4-star rating, which reflects above-average performance when compared to nursing homes across the country. That overall score is a composite picture built from three separate ratings, each measuring something different. The health inspection rating comes in at 4 stars, meaning state inspectors found fewer or less serious problems here than at most facilities - a good sign that the home is being run safely and is holding up well under outside scrutiny. The staffing rating is also 4 stars, which tells you nurses are spending more time with residents than the national average. More hands-on time generally means faster responses, more attentive care, and less chance of things being missed.
The one area worth keeping an eye on is the quality measures rating, which sits at 3 stars, or average. This score is based on 15 clinical indicators - things like how residents are doing with mobility, pain management, infections, and other health outcomes. Average is not a red flag, but it does mean the facility is not quite standing out in this area the way it is in staffing and inspections. When you visit, it would be reasonable to ask the staff how they track and improve resident health outcomes over time. Overall, the 4-star composite suggests this is a solid, above-average choice, with real strengths in care consistency and oversight, and some room to grow on the clinical outcomes side.

Questions to ask when visiting
1. How many residents does each certified nursing assistant care for during a typical day shift and a typical night shift?
2. How long have most of your nursing staff been working here, and what does your turnover rate look like over the past year?
3. If my loved one has a concern or feels unsafe, who do they talk to, and how quickly can we expect a response?
4. What happens on a regular weekday afternoon for a resident who does not have any scheduled therapy or activities?
5. Can you show me the most recent state inspection report, and walk me through anything that was cited as a deficiency?
6. If my loved one's condition changes suddenly at night or on a weekend, what is the process and who makes the call about next steps?

About CMS Star Ratings
CMS assigns 1–5 star ratings to Medicare and Medicaid-certified nursing homes based on three domains: health inspections, staffing levels, and quality measures. The overall rating is a weighted composite.
A 5-star overall rating does not guarantee excellence in every domain — inspect each sub-rating independently. Data is updated quarterly.
