Staffing Rating
St. Ann's Community holds a 4-star overall rating from CMS, which means it performs above average compared to nursing homes across the country. That overall score is built from three separate ratings, each measuring something different. The health inspection rating of 4 stars tells you that state inspectors found relatively few problems during their visits, and nothing too serious - a good sign that the facility is being run with care and accountability. The staffing rating of 4 stars means nurses are spending more time with residents than you'd see at a typical facility, which matters a lot for day-to-day comfort and how quickly concerns get addressed. The quality measures rating of 4 stars reflects strong outcomes across 15 clinical indicators - things like how well the facility manages pain, prevents pressure sores, and supports residents' physical functioning. When all three pieces come together at 4 stars, it paints a fairly consistent picture of a facility that's doing well across the board. There's no single area pulling the overall score down, which is reassuring. A 4-star rating won't tell you everything - a visit in person still matters - but it's a meaningful signal that St. Ann's is performing above the national average in the areas families tend to care about most.

Questions to ask when visiting
1. How many residents does each certified nursing assistant care for during the day shift, and how does that number change at night and on weekends? 2. How long have your charge nurses and CNAs been working here on average, and what does your staff turnover look like over the past year? 3. If my loved one falls or has a medical emergency in the middle of the night, walk me through exactly what happens from the moment staff is alerted to the moment a family member is contacted. 4. What does a typical weekday look like for a resident who is fairly mobile and alert, from the time they wake up to the time they go to bed? 5. Can you show me the most recent state inspection report for this facility, and how did you address any deficiencies that were cited? 6. If my loved one has a complaint or feels their needs are not being met, what is the process for raising that concern, and who outside of this facility can they contact if they feel the issue is not resolved? About CMS Star Ratings

CMS assigns 1–5 star ratings to Medicare and Medicaid-certified nursing homes based on three domains: health inspections, staffing levels, and quality measures. The overall rating is a weighted composite.

A 5-star overall rating does not guarantee excellence in every domain — inspect each sub-rating independently. Data is updated quarterly.
