Staffing Rating
Brookside Multicare Nursing Center holds an overall 4-star rating from CMS, which puts it above average compared to nursing homes across the country. That overall rating is a composite built from three separate scores, and in this case, all three areas line up consistently, which is a good sign that the facility is performing well across the board rather than excelling in one area while struggling in another.
Each piece of the rating tells you something different. The 4-star health inspection score means that when state surveyors visited and reviewed the facility, they found fewer or less serious problems than most nursing homes, so families can feel reasonably confident about safety and compliance. The 4-star staffing score reflects that nurses and aides are spending more time with residents than the average facility, which tends to translate into better day-to-day care and faster response to resident needs. And the 4-star quality measures score looks at 15 clinical outcomes, things like how residents are managing pain, whether they are developing pressure sores, or how mobility is holding up over time. Scoring above average here suggests residents at Brookside are generally experiencing better health outcomes than at most comparable facilities.

Questions to ask when visiting
1. How many residents does each certified nursing assistant typically care for during the day shift, and does that number change at night or on weekends?
2. How long have your charge nurses and CNAs been working here, and what does your turnover look like over the past year?
3. If my loved one had a fall or a sudden change in condition at 2 in the morning, walk me through exactly what would happen from the moment staff noticed something was wrong.
4. Can I see the most recent state inspection report, and can you tell me what you did to address any deficiencies that were cited?
5. How do you make sure residents who need help eating actually get it, and how do you handle someone who starts refusing meals or losing weight?
6. What does a typical Tuesday look like for a resident who is mobile and alert, and how is a typical day different for someone who stays mostly in their room?

About CMS Star Ratings
CMS assigns 1–5 star ratings to Medicare and Medicaid-certified nursing homes based on three domains: health inspections, staffing levels, and quality measures. The overall rating is a weighted composite.
A 5-star overall rating does not guarantee excellence in every domain — inspect each sub-rating independently. Data is updated quarterly.
