Staffing Rating
Berkshire Nursing & Rehabilitation Center holds a 5-star overall rating from CMS, which is the highest possible score. That overall rating is a composite of three separate scores, so it helps to look at each one individually to get the full picture. On health inspections, Berkshire earns 4 out of 5 stars, meaning state inspectors found fewer or less serious problems than most facilities. That's a good sign that the facility is being run safely and that care standards are being met. On quality measures, Berkshire scores a perfect 5 out of 5 stars, which reflects strong outcomes across 15 clinical indicators like infection rates, falls, pressure ulcers, and how well residents maintain their physical function. That's the piece of the rating that most directly tells you how residents are actually doing day to day. The one area worth paying attention to is staffing, which comes in at 2 out of 5 stars, meaning nurse hours per resident are below average compared to other nursing homes. For families, that can translate to longer wait times for assistance or less one-on-one attention for residents with more complex needs. It doesn't cancel out the strong quality and inspection scores, and the overall 5-star rating reflects that the facility performs well in important areas. But if your loved one needs a higher level of hands-on care, it's a good question to ask Berkshire directly when you visit.

Questions to ask when visiting
1. How many residents does each certified nursing assistant typically care for during the day shift, and how does that number change at night and on weekends? 2. How long have most of your nursing staff been working here, and what do you do when you have an open shift - do you use agency nurses or staff from within the building? 3. Can you walk me through exactly what happens when a resident falls or has a medical emergency, from the moment it occurs to when the family is contacted? 4. What does a typical weekday look like for a resident who needs help with daily activities - what time do they wake up, when do they eat, and how much of their routine do they actually get to choose? 5. What were your last few inspection results, and can you show me the most recent report along with what specific steps you took to fix any problems that were cited? 6. If my family member had a concern or complaint about their care, who exactly would they talk to, and can you give me a real example of a complaint that came up here and how it was resolved? About CMS Star Ratings

CMS assigns 1–5 star ratings to Medicare and Medicaid-certified nursing homes based on three domains: health inspections, staffing levels, and quality measures. The overall rating is a weighted composite.

A 5-star overall rating does not guarantee excellence in every domain — inspect each sub-rating independently. Data is updated quarterly.
