Staffing Rating
Orchard Manor holds a 5-star overall rating from CMS, which is the highest possible score and puts this facility well above average compared to nursing homes nationwide. That top-level rating is shaped by three pieces working together: health inspections, staffing, and quality measures. Orchard Manor earned a perfect 5 stars in both health inspections and staffing, which carries significant weight in the overall score. The health inspection rating means state surveyors found very few, if any, citations during their visits, and the ones they did find were minor. That's a strong sign that the facility is being run cleanly and safely day to day. The staffing rating tells you that nurses and aides are spending more time with each resident than you'd typically see, which matters a great deal when your loved one needs hands-on care and attention. The one area worth noting is the 3-star quality measures rating, which lands right at average. This score is based on 15 clinical indicators like how often residents experience falls, pressure sores, or changes in mobility. Average isn't bad, but it does mean there's room for improvement in some health outcome areas compared to the best-performing facilities. For most families, the combination here is genuinely reassuring. Two of the three pillars are as strong as they get, and the overall 5-star rating reflects that Orchard Manor is performing at a high level. The quality measures score is worth asking the facility about directly, just so you have a clear picture of where they're focused on getting even better.

Questions to ask when visiting
1. How many residents does each certified nursing assistant care for during the day shift, and how does that change at night and on weekends? 2. How long have your charge nurses and CNAs been working here, and what does your staff turnover look like over the past year? 3. If my loved one has a concern or feels unsafe, who do they talk to, and can you walk me through what happens after that conversation? 4. What does a typical day look like for a resident who doesn't have many scheduled therapies or activities - how are they spending their time? 5. How often does the same staff member work with the same residents, or does everyone rotate through different assignments regularly? 6. Can you show me the most recent state inspection report, and is there anything in it you want to explain or give me context on? About CMS Star Ratings

CMS assigns 1–5 star ratings to Medicare and Medicaid-certified nursing homes based on three domains: health inspections, staffing levels, and quality measures. The overall rating is a weighted composite.

A 5-star overall rating does not guarantee excellence in every domain — inspect each sub-rating independently. Data is updated quarterly.
